How to fill in a CMR consignment note
The CMR has 24 numbered boxes. Here is what each one is for and what to write in it. The layout can differ slightly between printed forms, but the numbering is standard.
Copies and signatures
The CMR is normally issued in three original copies: the first (red) stays with the sender, the second (blue) travels with the goods and goes to the consignee, the third (green) stays with the carrier. It is signed by the sender and the carrier; the consignee confirms receipt in box 24.

Practical tips
- Write clearly and use the same company names as on the invoice.
- Check the gross weight and number of packages against the loading list before signing.
- Note any damage or shortage in box 18 (carrier) or box 24 (consignee) at the time, not later.
- Keep a copy of every CMR; it is your proof of delivery.
